Application Addendum for Criminal History Record Information
Request - LISD is required by law to review the criminal
history of applicants, employees, student teachers, and
volunteers. The information requested on this form is necessary
to obtain criminal history record information.

Addendum for Bus Drivers and Coaches - Each person who
applies to be a bus driver must provide the information in this
form at the time of application. Note : Bus drivers must pass a
